What to do when a toothache starts at night

A plain-English guide to triaging tooth pain after hours, what can usually wait until morning, and what should not wait.

First decide whether it is urgent tonight

Tooth pain feels bigger at night because there is less else competing for your attention. The useful question is not whether it hurts. The useful question is whether there are signs that the problem could be spreading or needs urgent medical care.

  • Seek urgent medical help if swelling affects breathing or swallowing.
  • Seek urgent medical help for facial swelling with fever, feeling very unwell, or rapidly worsening symptoms.
  • Seek urgent help after trauma with uncontrolled bleeding or a knocked-out adult tooth.
  • Call the clinic during practice hours for toothache, a broken tooth, a lost filling, or pain that is not settling.

If it can wait until morning

If there is no swelling, fever, trauma, or breathing concern, the practical path is usually to protect the tooth, avoid chewing on that side, use pain relief only as directed on the packet if it is suitable for you, and call us when reception opens.

What happens when you call

Reception will ask what you are feeling, when it started, whether there is swelling or trauma, and whether you can come in that day. The first visit may focus on diagnosis and stabilising the problem rather than finishing every part of treatment immediately.
